Student Accident Insurance

The Sag Harbor School District does carry student accident insurance. This is a supplemental policy to the student’s own health insurance, and carries a $250 deductible per incident.

If a student gets injured on school grounds, or while participating in any school organized activity, you must follow the requirements of your insurance carrier for obtaining benefits, for example, seeking “treatment within network,” otherwise, this supplemental benefit may be reduced.

Treatment must be sought, and the student accident “Notification of Injury” form must be filed within 90 days of the injury, or the claim will be denied. Please see Claim Instructions below.
